Knowing people that can’t use a mouse, but can use a joystick (like on their chair), Skippy set out to build a cheap system for input. He’s done pretty well and put down a TON of documentation on the process.
The Wiimote was very interesting, and I haven’t seen many mods for it. This might be the only one out there. The buttons have been extended for people with fine motor control issues.
From the Hackday archives, here’s a great example of a cheap custom mouse joystick. They started with a standard USB controller and basically extended the buttons to connect to these easy access arcade buttons. While the cost of the parts wasn’t much, there’s several hours of work involved.
