I decided to share this as the principles and requirements are very similar to yours and the technology could be adapted with no real cost.

I made a recorder (sorry I don't know if you call it this in the US - it's a flute-like musical instrument) for an 8 year old girl who was born with only two digits on each hand. This instrument normally requires a full compliment of digits.

A manufacturer heard what I was trying to do and sent me 6 instruments to experiment with. I hollowed one on the lathe, made a PCB to fit inside it with 8 buttons along each side. The opposing pairs of buttons can be pressed with two fingers while the instrument is he;d with the other two. These button pairs are converted (by a Microchip PIC) into a MIDI stream and sent to a keyboard where they're converted to audio.

An interesting project for an excellent reason!
I am currently setting up an electronic wind controller with an mbed controller, capacitive touch sensors and a blow pressure sensor.
The pressure sensor has been the most difficult piece but now it is starting to work.
